About your Veterinarian

We need to contact your veterinarian to ask about the animals you have now, if any.

Please authorize your veterinarian to talk with us about your animals.
Your application will be processed faster if you will send pictures of your home inside and out. The pictures are because we can not do home visits at the present time. You can send them to rescuecharliesfriends@gmail.com or attach them to the automated reply you will receive when you submit this form.

After pressing "Submit" to send the form to us you will receive an email confirmation WITH INSTRUCTIONS ON WHAT TO DO NEXT , including a completed copy of the form which you can save or print for your records.

If you don't receive it, please check your spam folder.

Once approved you will receive an email letting you know that you have been approved for adoption. You will then be contacted by our team by email and/or phone to discuss the dogs or puppies you are interested in that are available.

After we find the perfect new family member for you, we will ask you to make a $100 deposit via the website. Please do NOT make a deposit until requested to do so.

When the deposit is received, we change the dog’s status to “Adoption Pending”. This reserves your dog and prevents someone else from adopting it.

Please read our Adoption Policy and fill out our Adoption Application.

We know that it may seem like a bit much, but please understand that we are trying to ensure that the dog has a loving environment and a happy, healthy future.

Once you submit the form, a copy of the completed form will be emailed to you for your records.

After your application is approved, we will ask that you make a deposit of $100.00 to reserve the Rescue Dog you want to adopt.  The balance must be paid before taking your new dog home.

Please do not make the deposit of $100.00 until after your application is approved.

After he or she is spayed or neutered and current on all vaccinations our Foster Coordinator will arrange to meet with you so you can take your new Best Friend home.
